Baker Chiropractic and Acupuncture, LLC offers our patient form(s) in an online format so they can be completed in the convenience of your own home or office.
Please carefully read the following instructions to ensure that you get the full amount of time for your new patient appointment and first treatment:
- If you do not complete and submit your New Patient Paperwork 48 hours ahead of the starting time for your appointment, we will have to reschedule your appointment
- If you need to cancel or reschedule, please call or message us on our website at least 24 hours in advance to kindly allow another patient in need to receive care, and to avoid being charged for the missed appointment. We often have a list of patients that need an appointment if a cancellation occurs.
New Patient Intake Forms & Directions to Our Office
Most often, you will have an email sent to you at the time of confirming your new patient appointment that will have a link and a password in it that you will follow to complete your new patient paperwork on a cell phone, tablet or computer. You will not need to download any paperwork or remember to bring it with you to the office.
If you have lost that email, feel free to text us and we can resend it to you.
If neither of those options are working for you, please Click HERE to be directed to our online New Patient Paperwork for All New Patients (except Lifeline Only Clients). You will then be asked for your Name and Email address, and sent a secure link to fill out your paperwork and submit it to us online, without needing to print it or remember to bring it with you. Our New Patient Paperwork can be completed on a computer, tablet or cell phone. Please text us if you have any issues accessing our New Patient Paperwork.
